Value based health care: quality of life and equality

Background. Modern healthcare is characterized by a crisis of trust (52% public dissatisfaction in the Russian Federation) and an “ethical lag” in the implementation of NBIC technologies. The existing technocratic model, focused on quantitative indicators and the objectification of the patient, requires transformation towards a value-oriented and human-centered approach. Purpose. To provide a theoretical justification and develop a conceptual model of Empathic Biointegrative Healthcare (EBH) as an innovative paradigm that harmonizes high technologies with spiritual and moral values. Results. The concept of EBH, based on K. Wilber’s integral theory and the concept of “active empathy,” has been formulated for the first time. Within the framework of the model, the following have been developed: an original concept of Electronic Bases of Integrated Knowledge (EBIK), combining clinical data with a hierarchy of patient values; protocols for empathic communication in a digital environment (the “three V’s” rule); algorithms for shared decision-making based on biomarkers and personal priorities. Ethical regulators (justice, reversibility, transparency) have been defined to minimize the risks of techno-stratification when using transhumanist technologies. It has been established that the integration of spiritual and moral screening into clinical practice increases patient compliance and subjective well-being (PROMs). Practical implications. The results are applicable in the design of “clinics of the future,” the development of state strategies to reduce digital inequality in medicine, the creation of interfaces for medical AI systems, and in bioethics curricula. The EBH model allows for a transition from fragmented diagnostics to holistic health management, ensuring national biosecurity through the preservation of human essence in the era of digitalization.

Обоснование. Для современной системы здравоохранения характерен кризис доверия. Так, 52% населения РФ недовольны ее оказанием. Также выделяется и «этический лаг» при внедрении NBIC-технологий. Распространенная технократическая модель в основном ориентирована на количественные показатели и объективацию пациента и требует изменений в сторону ценностно ориентированного и человекоцентричного подхода. Цель – теоретическое обоснование и разработка концептуальной модели эмпатического биоинтегративного здравоохранения (ЭБЗ) как инновационной парадигмы, гармонизирующей высокие технологии с духовно-нравственными ценностями. Результаты. Впервые сформулировано понятие этического биоинтегративного здравоохранения, которое основывается на интегральной теории К. Уилбера и концепции «активной эмпатии». Разработаны: авторская концепция электронных баз интегрированных знаний, где клинические данные объединяются с субъективной иерархией ценностей пациента; протоколы эмпатической коммуникации (правило «трех В»); алгоритмы совместного принятия решений на основе биомаркеров и индивидуальных приоритетов. Определены этические регуляторы (справедливость, обратимость, прозрачность) для снижения рисков техно-стратификации при использовании современных технологий. Установлено, что соединение духовно-нравственного скрининга в клиническую практику повышает комплаентность и субъективное благополучие пациентов. Область применения результатов. Результаты применимы при проектировании «клиник будущего», разработке государственных стратегий по сокращению цифрового неравенства в медицине, создании интерфейсов медицинских ИИ-систем и в учебных программах по биоэтике. Модель позволяет перейти от фрагментарной диагностики к холистическому управлению здоровьем, обеспечивая национальную биобезопасность через сохранение человеческого начала в эпоху цифровизации.
